Program Summary


The Family Practice & Counseling Network (FPCN) provides comprehensive primary care, dental and behavioral health services. The network promotes resiliency and well-being among patients, staff and surrounding communities.

The Network consists of three (3) federally qualified health centers (FQHC) in Philadelphia:

  • Abbottsford Falls Family Practice & Counseling (Northwest Philly);
  • Stephen & Sandra Sheller 11th Street Family Health Services of Drexel University (North Philly); and
  • Health Annex (Southwest Philly).

Today the FPCN serves more than 20,000 patients annually and is a NCQA certified Patient-Centered Medical Home and recognized by HRSA as a National Quality Leader for Behavioral Health.

FPCN is restructuring its services to encompass full prenatal, and postpartum care across its 3 main sites (Abbottsford Falls, 11th St Family Services and the Health Annex). The Perinatal Program consists of services to include education on nutrition, exercise, childbirth preparation, breastfeeding, infant and self-care; and mental health and substance abuse therapy. These services will be provided by interdisciplinary teams of professionals led by nurse midwives and doulas, and will include behavioral health specialists, addiction counselors, social workers, and registered nurses. Additional services will include transportation assistance; lactation counseling; prenatal and parenting education; legal services; and complementary mind/body services, such as yoga, massage, and meditation. All staff will have training in anti-racism, justice, diversity, and inclusion (A-JEDI), trauma-informed care, and sanctuary model practices.

Position Summary


The Perinatal RN performs nursing and care coordination functions focusing on postpartum care through the first three years. They will work closely with the Prenatal RN, MAs, CNMs, Doula, Physicians and as well as the primary care, mental health, and dental providers. This position requires travel between our different health centers.

Essential Duties and Functions

  • Work collaboratively with the Perinatal Health Team to provide comprehensive postpartum care for all patients starting at 36 weeks or pregnancy:

*Facilitates all new postpartum patient appointments.

*Reviews all encounter forms for completion.

*Facilitates the ONAF form process.

*Ensures all orders, referrals, phone calls, etc. are completed in a timely fashion.

  • Manage postpartum patient flow within the Perinatal Care setting:

*Track deliveries and schedule postpartum and newborn home visits.

*Participates in follow-up and reminder calls/letters for all prenatal patients.

*Promotes Postpartum/Family Planning/Well Child visit compliance among postpartum patients.

  • Provide care management to all postpartum patients as deemed appropriate (ER use/hospitalizations) including coordinating follow-up labs and screening; tracking and following abnormal results.
  • Serve as the primary care coordinator between the patient and medical providers, behavioral health providers, pharmacy, dental, hospital-based service providers, shared services, home visiting agencies, and/or other specialists.
  • Provide services to postpartum and family planning patients of all ages, including phone/walk-in triage, patient education, counseling, family planning activities, coordinate appointments and referrals, assist in obtaining needed services and supplies.
  • Provide comprehensive, individualized health education to postpartum patients.
  • Provide comprehensive group education programs for postpartum patients (and family members).

*Ensure content of group sessions are consistently evaluated to ensure high quality and standards of care.

*Ensure measurable outcomes/data are collected and analyzed.

  • Develop appropriate health education materials and programs for use in the health center, in conjunction with the Perinatal Health Team.
  • Implement and maintain quality improvement activities related to postpartum and family planning, in collaboration with the Perinatal Health Team.
  • Develop and maintain relationships with community-based organizations for bi-directional referrals, care coordination, and social supports.
  • Triage medical questions related to postpartum issues in collaboration with the Patient Access Center.
  • Maintain accurate medical records within the Network's EHR Athena Practice
  • Participate in quality improvement efforts to monitor and improve patient outcomes including best practices and HEDIS measures.
  • Participate in outreach activities to increase awareness of growing Perinatal Services at FPCN including the future Birth Center.
  • Communicate and interact effectively and appropriately with patients, visitors, and colleagues.
  • Prioritize and organize tasks; smoothly transition between tasks as needed.
  • Respond quickly and appropriately in emergent situations.
  • Demonstrate adaptability to conditions and willingness to learn new procedures, etc. as required.

Essential Duties Addendum

  • Future Activities Include:

*Home visiting to newborns and their families (this part of the program is still in design)

*Participate in prenatal classes and other educational activities.

*Coordinate with Doula on outreach and home visits.

Qualifications


Minimum Qualifications:

  • A graduate of a school of professional Nursing (AD, hospital, or BSN); BSN preferred;
  • Active Pennsylvania Registered Nurse license
  • At least 2 years of experience
  • Ability to travel between COH sites
  • Current neonatal resuscitation certification
  • Current certification in Basic Life Support (accredited by the American Heart Association)
  • Prior experience in a primary care setting
  • Strong computer, organizational, and administrative skills
  • EMR experience
  • Commitment to work with underserved populations and ability to work in non-profit environments.
  • Demonstrated cultural competence in communicating with low-income populations.
  • Demonstrated ability to function effectively in a team.
  • Commitment to helping reduce the fear associated with birth and bring joy in the birth experience.

Preferred qualifications:


  • BSN preferred
  • Centering Pregnancy Certification
  • Prior Public Health Experience
  • Knowledge and prior experience of trauma informed care
  • Lactation Support

  • Medical office environment with adequate lighting and ventilation

Lifting Requirements

  • Light: exerting up to 20 pounds of force frequently, and/or negligible amount of force constantly to move objects. The use of arm and/or leg control requires force greater than sedentary, but worker still sits majority of time.

Covid-19 Vaccine Mandate: FPCN, a program in the City of Philadelphia, are required to follow the COVID vaccine mandate for healthcare workers per Philadelphia Department of Public Health. FPCN requires Healthcare Workers to be fully vaccinated. Fully vaccinated means it has been two weeks since the final dose of vaccine. Thus, we require vaccination prior to start for all new hires. This currently does not include boosters.
